THIRTY-FIRST ANNUAL REPORT of the MEDICAL OFFICR OF HEALTH, Eeing the Report for the Year 1886. TO THE VESTRY OF ST. PANCRAS. Gentlemen, As an introduction to the vital and sanitary condition of a district, a concise topographical account facilitates its comprehension. St. Pancras extends roughly from the Gate House, Highgate, at the northeastern corner of the Parish to the junction of Oxford Street and Tottenham Court Road at the south-western corner, averaging four miles in length and one mile in width. For Registration purposes it is divided into six sub-districts. The Kentish Town sub-district embraces all that portion of the Parish lying north of the Regent's Canal. The Tottenham Court and Gray's Inn Lane sub-districts occupy that portion south of the Euston Road, a line through Mabledon Place separating the subdistricts. The central portion of the Parish, between the Canal and the Euston Road is divided into three sub-districts. The Regent's Park, west of High Street and Hampstead Road; the Camden Town, north of Crowndale and Pancras Roads, and the Somers Town, south of these two roads. Thus the Registration sub-districts differ widely from the Wards. A useful sub-division of the Parish falls naturally into Northern, Central, and Southern, and this is the sub-division adopted for the Sanitary work of your Inspectors. The Vital Statistics might also conveniently be considered under these three sub-divisions, but this year the statistics have been, not without labour, completely calculated separately for each Registration Sub-district in order to compare the vital and sanitary condition of the sub-districts with one another. Geologically St. Pancras is situated on the London clay, excepting the Tottenham Court sub-district which is upon the gravel. The southern end of the Parish averages 80 feet above high-water mark, at the Canal it rises to 100 feet and at the northern end to 300 and 400 feet. 6 The Northern sub-division is open on the west side to Parliament Fields and contains the old and new grounds of Highgate Cemetery towards the east side. The Central sub-division has Regent's Park and Primrose Hill on the west side, and the Railway Termini and Depots on the east side. The Southern sub-division is most shut in. METEOROLOGY. The mean temperature for the year was 48 7 degrees, or 0 7 below the average of 45 years, the mean daily range being 15.6 degrees, and coincident with the average of 45 years. The mean degree of humidity was 81 per cent., or 1 below the average. The mean degree of barometric pressure 29.734 inches, or .024 below the average. The rainfall was 24.21 inches, or 1.85 below the average. On the whole, the year was slightly drier and cooler than the average. VITAL STATISTICS. POPULATION. In entering upon the second half of the decennial interval between the census of' 1881 and that of 1891, the difficulty of calculating the statistics proportionately increases. The distance of time since the actual enumeration of the population took place requires the more care in estimating this basis of vital statistics. The following numbers show, according to the Census Returns, the growth of population in the Parish during the three decennial periods since 1851 and an estimation for 1891:— Numbers. According to this method of calculation the annual increase in numbers would be 940, whereas if calculated on the actual rate of increase prevailing in the preceding decennium, viz., 6.67 per cent. it would be 1479. 7 The following numbers show the growth of population calculated according to the difference between natural increase and actual increase:— Tears. Seeing that there was an emigration from the Parish of the large percentage of 49.4 in the last decade, the rate of emigration cannot have increased to any greate extent, and I have placed it at the probable percentage of 55 which represents an actual annual increase of 1337. Calculating the growth of population according to the number of houses produces another resultant. After subtracting old assessments taken off the rate-books from new assessments placed on, the increase in the number of assessments calculated to the middle of the year are as follows: — Increase. 1881 2nd quarter 31 1881-82 225 1882-83 121 1883-84 62 1884-85 87 1885-86 47 573 At the Census of 1881 the total number of houses was 25943 of which 24701 were inhabited, and 1242 or about 5 per cent. uninhabited. Deducting 5 per cent. from 573 then 545 would represent the increase since 1881 or 104 per annum. 0 * Figures not ascertainable. There is no reason to expect but that the increase for 1886-90 will be the same as for 1881-85, especially as the natural increase of 1886 is as nearly as possible the same as that of 1885. 8 The number of persons per house is increasing, the following figures show this tendency:— Census. MARRIAGES. The number of Marriages celebrated in St. Pancras during 1836 was 2,128, this is 87 below the average of the preceding 10 years, but an increase of 13 upon the number in 1885. BIRTHS. The number of births registered in St. Pancras daring 1836 was 7,590, this is 391 below the decennial average, but 70 above the number of the preceding year. 186 births occurred in the Workhouse, and have been distributed to the subdistricts according to population. During the 52 weeks of 1886, ending January, 1st, 1887, the corrected number of deaths amounted to 4,877, equal to a rate of 20.15 per 1000 living The number was 117 more than in 1885, and 118 more than in 1884, in which latter year, and in the preceding years, the deaths of St. Pancras Parishioners dying in ExtraMetropolitan Lunatic Asylums were not added. In 1886 there were 67 such deaths, which if subtracted from 4877 reduces the number to 4810, and the deathrate by 0.28, that is to 19.87, the death rate in 1885, being 19.89, or less deaths in Extra-Metropolitan Lunatic Asylums 19.5, and in 1884, 19.72. The death-rate of all London daring 1886 was 19.9, and the average for the preceding 10 years of 11 St. Pancras, 20.7, and of London, 21.1. The number of Deaths from the principal Zymotic Diseases in St. Pancras was 616, the number in the previous year being 629. The number of deaths from violence was 179, of which 133 were due to accident or negligence, 6 to homicide and 10 to suicide. There is a considerable increase in the number of suicides, which during the last several years have averaged from 20 to 25. UNCERTIFIED DEATHS. The number of uncertified deaths in St. Pancras in 1886 was 4, against 11 in 1885, and 9 in 1884 and 1883. INQUESTS. 464 Inquests were held during the year, a number slightly below the average. 22 COMPARATIVE MORTALITY OF THE SUB-DISTRICTS. SANITATION. THE MORTUARIES. The next annual report, it is to be trusted, will contain an account of some useful work done by the Public Mortuaries for infectious and non-infectious dead. It can only be recorded at present that the Mortuaries together with the attached buildings approach completion. INFECTIOUS DISEASES. Notification.—The following number of infectious cases have come to the knowledge of the Sanitary Department from various sources:— Smallpox 12 Scarlatina 219 Diphtheria 60 Typhus Fever 0 Typhoid Fever 60 Continued Fever 2 353 Isolation.—The following number of cases were removed to the Metropolitan Asylums Board's Hospitals:— Smallpox 9 Scarlatina 129 Typhoid Fever 10 148 24 DISINFECTION. During the year 290 rooms were fumigated, after Infectious Diseases and notices served to strip and cleanse them. The following articles were stoved in the disinfecting apparatus:— Beds 123 Mattresses 61 Pillows 346 Sheets 133 Blankets 358 Counterpanes 122 Carpets 26 Wearing Apparel 336 Disinfection, after Infectious Disease, consists of the following process: 1. Fumigation of the air of the room, 2. Stripping, washing, and white-washing, the respective surfaces, and 3. Stoving the stuff materials, well boiling the linen and cotton, and washing the furniture. The washing water if mixed with the disinfectant recommended to your Works Department for use, viz. Soluble Creosote, will require no soap. The Stoving can only be effectually accomplished in a properly constructed disinfecting apparatus, such as your Vestry has provided for free public use. LIGHT AND AIR. Any causes obstructing the light or air, are injurious to the well being of both animals and plants, and above all of man. Density of Population increases in proportion to the density of dwellings, and of the density of individuals living therein, and the death rate of a district is in direct proportion to its density of population. In 1881, the density of the Metropolis was 54 persons per acre. „ „ St. Pancras was 88 „ In 1886, „ „ is 90 ,, In 1881 the density of Persons per acre in the sub-districts was— Regent's Park 93—including Canal and Parks. Tottenham Court 192 „ but little free space. Gray's Inn Lane 196 „ „ „ Somers Town 188 „ Railway Termini. Camden Town 102 „ Canal, Railway Terminus, and Gas works. Kentish Town 54 „ Canal, Railway Depots, and Fields. It must therefore be the care of St. Pancras to insure against increasing density, especially in the crowded parts of the southern sub-districts. Open Spaces, of small extent exist in various parts of the Parish, and should be allowed to continue to exist, not only as breathing spices, bat as recreation grounds 25 for the young, if the next generation is to retain its vitality. A small space in the centre of a crowded neighbourhood, is of greater utility and benefit, than a larger space at a distance. But an available open space even if situated at a distance is better than no space at all, and your Vestry is alive to the health of the parishioners in endeavouring fo repair the groundlost in by-gone years, by the prospect of securing Parliament Fields to the Public, and especially to the Public of St. Pancras. At present the public open spaces are as follows:— Regent's Park Primrose Hill Parliament Fields, not yet acquired St. Pancras Gardens, Pancras Road St. George's Burial Grounds St. Andrew's Burial Ground St. Martin's ,, ,, not yet acquired Whitfield „ „ „ „ Obstructed Dwellings separately or collectively may exist in a district which otherwise may be blessed with light and airy surroundings. Would that this had been borne in mind before the airy surroundings had disappeared, and the curtilages been allowed to be cut off! It is well to remember that as the larger open spaces are situated to the West and to the North of the Parish, the South and East, or Gray's Inn Lane sub-district, claims most attention, and the more so as it has been shown that the greatest density and the greatest mortality amongst young life occurs in this sub-district. Obstructed dwellings may be placed in three classes: 1.— Cellar Dwellings, obstructed principally from above. 2.—Back to back Dwellings, obstructed from behind, and when situated in a narrow passage from before also. 3.—Enclosed Dwellings, obstructed on all sides, of which Brunswick Grove is a dire example. It is easier to prevent the mistake of the present by the retention of open fields, than to repair the errors of a past generation by the reconstruction of areas such as exist in the Somers Town, and the Gray's Inn Lane sub-districts, and especially around Cromer Street. During the year 241 Cellar Dwellings were reported by your Inspectors, and visited by your Sanitary Committee and Medical Officer, of which 109 were ordered to be closed or amended, the principal offender being Little Albany Street. When it is remembered that the occupation as dwellings of bad examples of these kitchens or cellars is not only injurious to the inmates themselves, but also frequently deprives the other tenants in the house of the use of the basement, for washing and other purposes, the evil can be seen to be of widespreading effect, due to deprivation by the owner. It is therefore no hardship to compel the kitchen to revert to its proper or cognate usages, unless it is rendered truly fit for a dwelling place. The possession of two excellent sets of Baths and Wash-houses by the Parish, one central and the other south-west, can be no justification for this course of deprivation or the remedy would be worse than the disease. 26 As to enclosed and back to back dwellings, the only remedy of any permanent utility is the reconstruction of the blocks. This reconstruction can be carried out with least difficulty at or near the expiration of the original building lease, when the Freeholder's interest to improve his property coincides with the Sanitary Authority's interest to improve the health of those dwelling upon it. I am strongly of opinion that, where it is possible, a right course to pursue would be to ascertain the date of the expiration of the building leases, and just as the Freeholder compels the individual houses where necessary to be thoroughly repaired, so should the Sanitary Authority compel the blocks of dwellings where necessarv to be thoroughly re-modelled. The Houses of the Working Classes Act 1885, Sec. 4, absolves the Local Authority from the compulsory purchase of premises, ordered to be amended or demolished. Appended is a list of improved Industrial dwellings in St. Pancras, and it will be seen that the provision is small for a population approaching a quarter of a million and containing a large industrial class. 27 LIST OF IMPROVED INDUSTRIAL DWELLINGS IN ST. Purkins, Esq., 2, Cambridge Place, Regent's Park 1883 24 12 24 36 60 Totals 2197 757 1634 2175 3809 28 In the case of dwellings not requiring re-modelling in block but individual amendment, the lease having a greater or less number of years to run, sec. 25 of the Artizan and Labourers Dwellings Act of 1868, allows the Local Authority to make a charging order upon the inheritance, or fee of the premises improved by a leaseholder upon the requirement of the Local Authority. This is really compensation for improvement, not paid by the Vestry but by the freeholder or owner, according to the length of time expired before the improvement falls into his hands. Thereby your Yestry is able to act with little hardship to all concerned. The danger of this section is that apart from the natural decay of dwellings, the leaseholder may allow the premises purposely to fall into such a condition as to bring them under this compensation clause. The only means to prevent this to a great extent is for your Yestry to adopt Bye-laws for Houses let in Lodgings, a course now pursued by nearly all the London Vestries and District Boards. Tentatively, only those premises the owners of which are recurringly neglectful might be placed by your Sanitary Committee upon the Register as a precaution and a warning, by that means many of the benefits of the Artizans Dwelling Act may be obtained piecemeal, and extreme measures may be held in reserve for suitable cases, or when other means are exhausted. If only a limited number of premises were placed upon the Register for the purpose primarily of preventing recurring nuisances with the present system of constant bouse to house inspection, no immediate increase in the Staff of your Sanitary Department would be necessary for that purpose. In fact if recurring and other nuisances could be effectually kept in check by the stigma of being placed upon the Register, the work of your Inspectors would be materially facilitated. COMMON LODGING HOUSES. There are 29 Common Lodging Houses in the Parish, which are in a more sanitary condition than many of the tenements occupied by a class socially and morally more worthy of consideration, this is due to the power of supervision under proper regulations or bye-laws. A list of the Common Lodging Houses is appended to the end of the Report. CANAL BOATS. There was one prosecution during the year, for the infringement of the Cana Boats Regulations, viz.: against W. Studd, of Ivor, West Drayton, for non registration as a dwelling. CARAVANS AND TENTS. This class of dwellings does not much effect St. Pancras, but a caravan in Whitfield Burial Ground is at the present moment under observation. WATER SUPPLY. The New River Company has extended the constant supply system, as far North as a line midway between the Hampstead Junction, and North London Railways, and the whole of this Company's area in this Parish will be supplied on the constant system by the Autumn of 1887. 29 St. Pancras will be then entirely supplied by water on the constant system, excepting a small area served by the West Middlesex Company, situated West of the Hampstead Road, between Cumberland Market and the Euston Road. DRAINAGE. A record of the results of drain testing was only commenced in November of 1886, the figures for the two months are as follows:— No. Tested. RESULT. Satisfactory. Defective. The lower half of the Gray's Inn Road, a District which, as shown, has the highest mortalities in the Parish, suffers from a faulty system of sewerage. A properly laid sewer for this neighbourhood is an urgent question that requires serious attention. NUISANCES. Tables 7 and 8 give a summary of the nuisances abated and of other Sanitary work executed without and within dwellings. 7,127 premises were inspected, and 14,521 re-inspections made. VENTILATION AND OVERCROWDING. The model Bye-laws of the Local Government Board give 300 cubic feet, as a minimum for an adult, and half that amount for a child under 10 years of age. Parkes and other authorities agree that 1,000 cubic feet is the proper amount for an adult. When it is considered that persons both live and sleep in the 300 and 150 cubic feet minimum allowed, it is obvious that much depends upon the ventilation of the room, which your Sanitary Department has no power to regulate, and that a grievous injury is inflicted upon ignorant parent and offspring, by allowing confinement in any unventilated space of 300 and 150 cubic feet. The future health of individuals is thereby brought into such a condition as to unfit them for healthy labour, and to drive them prematurely as a burden on to the poor rates, directly and indirectly a doubly expensive remedy. In dwellings in which persistent overcrowding takes place it is impossible without Bye-laws to prevent its recurrence and provide for proper ventilation. REFUSE REMOVAL. Of the four kinds of refuse, house, stable, trade, and street, your Sanitary Department is more directly concerned with the two first. The vigilance of your Inspectors procures the constant regular removal of stable refuse, of which it is rare 30 to receive a complaint. It is otherwise with regard to house refuse or dust, and it would be impossible to inspect 25,000 houses as frequently as a few dozen stableyards, but the reason undoubtedly is the inability under the present method of securing constant regular removal. How this could best be secured, would require a long special report, but that it can be done is shown by several London Parishes, and St. Pancras would benefit materially in health, especially in the poorer and densely populated districts, by the more frequent removal of decomposing matter. There are 37 Licensed Slaughter Houses in the Parish, and during the year they have been kept in cleanly condition, and with due observance to the Bye-laws regulating them. COW SHEDS. There are 48 Licensed Cow Sheds in the Parish, they have been kept in cleanly condition and the regulations complied with. DAIRIES AND MILKSHOPS. The supervision of Dairies and Milk-shops, is not placed in the hands of the Sanitary Authorities in the Metropolis, as it is over the rest of the country, but in the hands of the Metropolitan Board of Works. 31 BAKEHOUSES. There are 214 bake-houses in the Parish; they have been kept in cleanly condition and in compliance with the regulations. MARINE STORES. There are 54 Marino Stores in the Parish, to which the attention of your Inspectors is directed to prevent the occurrence of any nuisance, which is very liable to arise upon premises where much decomposable matter is 6tored. It is a question whether they should not be classed as an offensive trade. LEGISLATION. Public Health (Metropolis) Bill 1885. This bill which, by codification, would have done for the Metropolis what the Public Health Act has done for the country generally, still remains in abeyance, together with other Metropolitan legislation. Sanitation of Houses (Metropolis) Bill, was a crude measure for the creation of new Authorities and new Officers, and had little hope of being adopted. Sanitary Registration of Buildings Bill, seeks to secure the sanitary registration of Schools, Hospitals, Hotels, etc., but by means that do not meet with general approval, the Sanitary Authority's power being reduced to a minimum, if not altogether superseded.
